(The provider will verify your eligibility and vision plan coverage and will request authorization for services and materials. If you are not currently eligible for services, the provider will let you know.)
- There are no claim forms to complete when you use a VSP provider.

Life and AD&D Insurance Overview
Ensign provides basic life and Accidental Death and Dismemberment (AD&D) coverage equal to your basic annual earnings (minimum benefit of $50,000) through Guardian at no cost to you. AD&D pays a benefit in addition to any life insurance if you are injured or die in an accident.
